The Problem Samaritan Saw

Frontline care teams use Samaritan to engage people in hospitals, shelters, and on the street. Each participant, or “Member,” receives a secure Samaritan Smart Wallet, sets personal goals, and gains 12 months of social and financial support to take action steps toward those goals.
These micro-steps lead to measurable transformation: Members complete up to 400% more care plan actions than traditional case management benchmarks, often resulting in stable housing and employment within a year.
Health systems deploying Samaritan have realized up to 5x ROI, reducing emergency utilization while increasing care coordination capacity.
